Yes, we charge a fee, due at the time forms are received by the office, for all paperwork that requires completion by any of our providers.
We also require 5-10 business days for the completion of any form. You will be contacted by our office once your paperwork has been completed.
Disability, FMLA, work or school accommodations, and miscellaneous forms are a standard fee of $25 up to 2 pages. Forms with more than 2 pages are subject to an additional charge of $5 per page.
DMV forms such as handicap placard or license plate applications and interval reports are $5.
Changes to any forms that have been completed and submitted are subject to an additional fee.
