Our patient paperwork is electronic only and will be sent to the email address and cell phone number provided at the time of appointment scheduling.
A link to appointment-specific paperwork is sent 72 hours prior to your appointment. If the paperwork is not completed, another link will be sent 24 hours prior to your scheduled appointment time.
Please note that the link with patient paperwork is a separate link sent from our online check in platform, Phreesia. You will not be able to access the link for paperwork from our patient portal. If you need to have the link for your appointment re-sent to you, please contact our scheduling department.
